Student Absence Notice 学生请假申请

A Student Absence Notice is requested in order to notify us when your child will not attend class. 如需请假，请填写该学生请假申请表。

Dismissal Change Notice 放学回家方式调整申请

A Dismissal Change Notice is required if your child’s dismissal procedure will change, for example if he will be picked up by someone other than the usual person, or will use a different bus stop. 如果您孩子放学回家的方式有变化，请填写该放学方式调整申请表。例如，家长临时安排他人（非家长本人）在学校或校车站点接孩子回家的情况，需要填写该表格。
